Register a Student

Are you planning to enroll a child in a Newark City District school?

Parents/caretakers should complete the following forms. These forms may be downloaded from this site. They also are available at each school. Parents/caretakers must bring the following information to the school at the time of registration:
  • Birth Certificate
  • Current Immunization Record
  • Child custody records, if applicable
  • Proof of residency (such as a paid utility bill, rental or lease agreement, etc.)
Parents/caretakers should contact the child’s physician, health department or school to determine if your child has receive the require immunizations. The record of immunization must be on file with the school before the first day of class, or, by law, the child must be sent home. The Newark City Health Department holds regular immunization clinics for children. Interested parents should immediately contact the Health Department, 349-6680, for an appointment.

When all forms and documents are ready, the parent/caregiver may wish to set an appointment with the school to ensure the fastest service. However, families who walk in without an appointment will be accommodated.
